1. Increased airway hyperreactivity with the M40 protective mask in exercise-induced bronchospasm

Objective:Exercise-induced bronchospasm (EIB) has a prevalence of 6% to 7% in United States Army personnel and 3% to 13% in professional athletes. There are reported concerns that military personnel with EIB will have increased airway hyperreactivity or significant dyspnea while wearing the standard military M40 protective mask. The objective of this study is to determine whether the M40 protective gas mask increases airway hyperreactivity in military personnel with exertional dyspnea and the diagnosis of EIB.Methods:Ten active duty military with EIB (defined as history of exertional dyspnea, normal spirometry, and reactive methacholine challenge test) and 10 normal control subjects were evaluated. Both the participants and control subjects underwent baseline exercise challenge testing (ECT) with and without the M40 protective mask. Forced expiratory volume in one second (FEV1) (percent predicted) post ECT was compared to baseline FEV1 within and between groups along with exercise time.Results:There was no statistical difference in between individuals and between groups wearing the M40 mask. None of the study group had a positive ECT exercising without the M40 mask while 20% of the study group with EIB had a positive ECT wearing the M40 mask.Conclusion:Military personnel with EIB who exercised with the M40 protective mask did not overall have significantly increased airway hyperreactivity compared to control subjects. Screening ECT may be beneficial in identifying those susceptible persons who report symptoms while wearing the M40 protective mask.

Rising healthcare complications due to fungal infections increase the importance of efficient specimen collection and maintenance systems for correct identification and diagnosis. The CLSI M40-A2 protocol provides guidelines for laboratories assessing quality of medical transport devices, including swab transport systems (STS). This study assessed the efficiency of the Sigma Transwab® foam and flock swab in recovering and maintaining viability of different Candida spp. including C. auris, in different test conditions. Both swab types recovered and maintained viability of all Candida spp. with greater CFU at room temperature after incubation (24 and 48 h) in comparison with swabs maintained at 4 °C.
3. In Vivo Replication and Pathogenesis of Vesicular Stomatitis Virus Recombinant M40 Containing Ebola Virus L-Domain Sequences

The M40 VSV recombinant was engineered to contain overlapping PTAP and PPxY L-domain motifs and flanking residues from the VP40 protein of Ebola virus. Replication of M40 in cell culture is virtually indistinguishable from that of control viruses. However, the presence of the Ebola PTAP motif in the M40 recombinant enabled this virus to interact with and recruit host Tsg101, which was packaged into M40 virions. In this brief report, we compared replication and the pathogenic profiles of M40 and the parental virus M51R in mice to determine whether the presence of the Ebola L-domains and flanking residues altered in vivo characteristics of the virus. Overall, the in vivo characteristics of M40 were similar to those of the parental M51R virus, indicating that the Ebola sequences did not alter pathogenesis of VSV in this small animal model of infection.
